Transition Program

Welcome to the Weigelli Centre Alcohol & Other Drugs (AOD) Transitional Program Model—a vital bridge between intensive residential rehabilitation and independent community living.

Designed specifically for residents who have successfully completed our 12-week residential therapeutic program, this transitional model provides the structured, step-down support needed to prevent relapse, secure employment, and build a foundation for sustainable, long-term recovery.

Core Pillars of the Program

Our model is built on four interconnected pillars to ensure a holistic and supported transition:

Step- Down Accommodation

Clients move into supportive, substance-free transitional program for 3 to 9 months after completing initial 12 week program, (reviewed quarterly), offering a stable environment during a critical phase of recovery.

Therapeutic & Case Management Support

We ensure continuity of care through peer group therapy, individual clinical counselling, goal-oriented case reviews, and warm referrals to external specialist health, legal, and social services.

Harm Reduction & Abstinence Strategies

All interventions are grounded in culturally safe, trauma-informed care. We integrate targeted educational groups, relapse prevention sessions, and SMART Recovery groups.

Community Reintegration & Life Skills:

We empower clients through financial literacy and budgeting assistance, vocational training, employment and volunteering pathways, family reunification support, and help securing permanent housing.
